If you are a new client or have a new pet addition to the family please fill out our New Patient Form for your next visit. You may bring this form with you to your next appointment, email or fax it to our office at your convenience.
If your pet is scheduled for surgery, you will be asked to review and sign a Surgery Consent Form on the morning of the surgery. In order to save time, we recommend our clients review this form before the morning of surgery.
Clients will receive a phone call the day before surgery to discuss food and water restrictions for your pet. Typically we recommend no food past 7 p.m. the night before surgery. Your pet may have water up until the morning of the surgery at 8 a.m. However, each surgery is tailored to an individual pet's needs so be sure to check what the specific requirements are for your pet.
